Contract: permanent Hours of work: 36 hours per week Location: Civic Centre and other locations from time to time The SMART Service (Supportive Multi-Agency Response Team) is looking to appoint a dedicated Occupational Therapist for this brand new and innovative team Brent is a tremendously vibrant London borough where the iconic arch of Wembley Stadium dominates the skyline. The Post This is a new and unique service in the London Borough of Brent and has been created to provide targeted support for individuals with intersecting complex needs, particularly those with vulnerability and experience of exclusion that impacts on their mental health and wellbeing. These individuals would not normally be eligible for support from adult social care, as they do not meet the criteria set out in the Care Act. They may be under primary care for mental health, and are less likely to be managed long term under secondary mental health, potentially due to their reduced ability to engage. The SMART Service is a creative response to work with people within the Borough who often have a cycle of homelessness, mental health admissions, drug and alcohol dependencies and contact with Criminal justice and probation services. Their multiple complex needs requires a more holistic response in order for them to be able to move forward and until now there has not been a targeted provision for this group of people. The Person We are looking to recruit a dedicated Occupational therapist as part of a small multidisciplinary team that includes two Drug and Alcohol practitioners, a housing worker, an occupational therapist, an Advanced Practitioner, a Social Worker and two Support Outreach Officers. We are looking for an Occupational Therapist who is passionate and has experience of working effectively with people with dual diagnosis and complex social needs and who is creative about how to assess and support this group of marginalised people. Utilising your strengths in communication and engagement you will support service users who traditionally have found it difficult to maintain a relationship with professionals. You will be able to work alongside service users in order to assist them to make progress and enhance their independence with their ADL’s. You will be able to develop and maintain close professional relationships within the Local Authority and work closely with other homeless network agencies, charitable organisations and Voluntary and community groups. You will be able to work creatively with an assertive outreach approach. You will enjoy working closely within a small team to maximise the opportunity for change for the service user.
